Organizational Background
Pinnacle Emergency Services is a privately operated EMS provider with 180 employees covering a two-county region. Eight months ago, a multi-vehicle accident resulted in two patient deaths that a post-incident review attributed partly to a communication breakdown between crews. The three paramedics involved received protocol retraining and returned to full duty within 30 days. There was no psychological debriefing. There was no organizational acknowledgment of the trauma experienced by first responders. In the eight months since: six paramedics have resigned, absenteeism has doubled, incident reporting has declined sharply (a known indicator of psychological safety erosion), and two of the three involved paramedics are showing observable signs of PTSD. The CEO believes the protocols have been fixed. The workforce believes nothing has been fixed - because the thing that broke wasn't a protocol.
